How do I qualify for an Ohio Medical Marijuana card?

In order to qualify, you will need to submit a photo of your Ohio ID and medical records or a note from your doctor verifying that you have been diagnosed with one of the 21 qualifying conditions in the state of Ohio. You can find the conditions LOCATED HERE on our website.

What if I don’t have an Ohio ID?

You must be a resident of Ohio in order to qualify for Ohio’s medical marijuana program. If you are out of state, you will need to find a program within your state. If you are a resident of Ohio and do not have an Ohio driver’s license or ID, you can provide a US Passport with two proofs of Ohio residency (ex. utility bill, bank statement) as an alternative. Keep in mind that if you choose to use your passport, you will need to bring your passport and the two proofs of residency with you every time you go to the dispensary. If you have just changed your Ohio residency and are waiting for your physical Ohio ID to come in the mail, we can accept your temporary paper ID from the Ohio BMV.

I have been diagnosed with at least one of Ohio’s qualifying conditions, how can I show you?

You will need to either submit a note from your physician (MD or DO) or medical records verifying your qualifying condition(s). Documents can be faxed directly to us at (614) 388-9344. If you have online access to your records, call or text us at (614) 437-8382 so we can walk you through the process of sharing your records with us.

I don’t have access to any of my medical records, what should I do?

We can email you a medical records release form to securely complete online. Once complete, we will fax it to your doctor to request records on your behalf. If you require one of these forms, let us know by leaving a note when you schedule online, or texting your first and last name + email address that we can send the form to at (614) 437-8382. You can also email us requesting a form at info@releafhealthclinic.com.

How much does this cost?

New patient consultations are $149 and renewal consultations are $75 for a full 360 day recommendation upon approval. All patients are responsible for paying the annual $50 state fee to activate or renew their medical marijuana cards unless they’ve opted for assisted registration (additional $50 fee). Email instructions to pay the state fee will follow after the appointment once the patient has been approved. If not approved, we issue a FULL refund per the consultation fee.

It’s time to see the doctor, what should I expect?

By now, you should have already submitted your Ohio ID and medical records (or completed a medical records release form). The doctor will text a link to your smartphone once they are ready for you. Simply tap the link to enter the video call! They do NOT send the link in advance. Once you have completed your appointment and been approved, you will receive email instructions on how to complete the registration process, pay your state activation fee, and download or print your MMJ card!

When can I go to the dispensary?

Once you’ve printed or downloaded your MMJ card, you can take it to any Ohio dispensary right away. Be sure to bring your Ohio ID with you! You can view a list of dispensaries and browse their menus online or find a dispensary near you at www.weedmaps.com.

What am I allowed to purchase in the dispensaries?

You are not limited to what products you can buy, only how much you can buy. The state of Ohio limits each patient to purchasing a 45 day supply at a time. When you are inside the dispensaries, the workers will let you know which products are worth how many days. Every 45 days, your product days will “restart” until your full 360 day recommendation is up. Days do NOT roll over.

How do I know which products to buy?

Medical marijuana will be subjective for each patient. Our doctors recommend keeping a journal and logging which product you’re consuming, how much of it you’re consuming, and even what time of the day you are consuming it. We recommend starting at small doses and working your way up as needed. Keep in mind that some products will have longer onset times than others, patience is key!

The dispensary said my recommendation is expired but my medical marijuana card says it doesn’t expire until the end of the month?

There are two separate components to your medical marijauana card: the doctor’s recommendation and the card itself. The doctor’s recommendation acts as the prescription on your MMJ card and is good for exactly 360 days. The medical marijuana card is the holder for your prescription and will always round out to the end of the month that you activated it in. You must have BOTH an active recommendation and active medical marijuana card in order to gain access to the dispensary. If your recommendation expires, you will need to schedule a renewal consultation with one of our doctors.

How often do I need to renew my card?

You will need to renew your recommendation and card once a year. Your recommendation will typically expire before your card because it is good for 360 days exactly. You do NOT pay any penalty for letting your recommendation or card expire. Once you have had your Ohio Medical Marijuana Card, you will always be considered a renewing patient! To schedule a renewal consultation to obtain a new recommendation, you can select a time slot for your telemedicine appointment online. We will email you instructions on how to pay the annual state fee to renew your MMJ card itself once you’ve been approved for your new recommendation!

Are there any discounts?

There are discounts available for those who are Veterans or Indigent status. If you are a Veteran, you will need to submit either your Veteran ID or DD214 showing the character of discharge. If you are of Indigent status, you will need to submit proof of monthly disability income for the current year.

What website do I go to view my patient information?

You will go to www.ohiomedicalmarijuanaregistry.com and log in using your FULL email address as your username. If you do not know your password, there will be an option to email yourself a password reset link by clicking RESET PASSWORD. Here, you can view the details of your recommendation, your recommendation expiration date, your MMJ card expiration date, and your product fill periods.

I have difficulty leaving my home, can someone pick up the product for me?

Each patient is allowed to have up to two caregivers that can go into Ohio dispensaries and purchase medical marijuana products on their behalf. Please email us at info@releafhealthclinic.com to request caregiver registration information.
